Care Plan Timing and Revision
The comprehensive care plan must be reviewed and revised by the interdisciplinary team after each assessment and as the resident's condition changes.
How in-house dermatology helps
Each visit returns dated findings and a recommended follow-up interval, giving the IDT concrete content to use when it reviews and revises the care plan. The revision itself stays with the interdisciplinary team.
We are not a wound care service and we do not replace your wound team. We work alongside them as another set of specialty eyes, catching dermatologic issues early — including the non-healing areas that turn out to be skin cancer, atypical infection, or inflammatory skin disease rather than a wound.
What surveyors look at
- Whether the care plan was revised after a significant change
- Whether the IDT documented its review and the resident or representative was involved

- A new diagnosis in the progress notes that never reaches the care plan
- A care plan unchanged across multiple quarters despite documented skin changes
